Mail Packet
Send your fingerprint cards and the following required documents to Capital Live Scan: copy of valid ID (front and back), signed Declaration of Individual Rolling Fingerprints, payment (Credit Card Authorization Form or check), and completed Request for Live Scan Service (BCIA 8016) — provided by the requesting agency. Make sure all forms are complete before mailing to avoid delays.
Convert to Live Scan
We will convert your fingerprint cards to a Live Scan transaction and submit them electronically to the CA DOJ and/or the FBI.
Deliver Results
Your background check results are delivered just like a Live Scan submission from the CA DOJ and FBI to the applicant agency authorized to receive your background check results.

A completed Request for Live Scan Service (form BCIA 8016) - you will get this from the agency requesting your background check.
Photo I.D. Copy
A copy (front and back) of your valid Photo I.D.
